Vitiligo is a disease of subtraction. Melanocytes are progressively destroyed, and what remains is well-demarcated depigmented skin against normally pigmented surroundings. Vitiligo is disfiguring in the specific sense that matters clinically, visible and unpredictable, and the measured burden falls on quality of life rather than on physical function. It is also worth stating plainly that repigmentation in vitiligo requires melanocyte repopulation from follicular reservoirs, a process that established phototherapy and immunomodulatory approaches are designed around and that no nutritional intervention has been shown to drive.
