What separates rosacea from acne is what it lacks. Both produce papules and pustules on the central face, but rosacea produces no comedones, and that absence points at an entirely different mechanism. Where acne begins in a plugged pilosebaceous duct, rosacea begins in vascular and innate immune dysregulation.
