Combination formulas for hair, skin and nails constitute one of the largest supplement categories by revenue, exceeding two billion dollars annually, and the grouping has a real biological basis even where the marketing outruns it. All three tissues are keratinised epithelial derivatives with overlapping requirements: structural keratin, the cofactors of its synthesis, and the collagen scaffolding of the surrounding dermis and nail matrix. Three ingredients carry an evidence grade under this heading, two at Grade C and one at Grade D, across 14 studies and 880 participants.
The grouping also explains why deficiency shows up in all three at once. Brittle nails, thinning or dull hair and rough skin texture commonly appear together during nutritional shortfall, crash dieting, hormonal transition or sustained stress, because the body deprioritises tissues that are not immediately essential. That co-occurrence is the strongest argument for grouping them as one category, and it also means that visible improvement in a deficient person says little about what happens in a replete one.
Biotin holds Grade C on 8 studies and 540 participants, the widest base in the set. It functions as the coenzyme for carboxylases central to fatty acid synthesis and amino acid metabolism, and genuine biotin deficiency produces alopecia and brittle nails that resolve on repletion. The gap between that fact and the general use of biotin is where the evidence thins, since frank deficiency is rare in adequately fed populations. Formulations pairing cystine with thiamine have been examined for their effect on keratinocyte responses under growth-limiting conditions [1].
Silicon holds Grade C on 4 studies and 220 participants, with orthosilicic acid proposed as a cofactor in collagen synthesis and connective tissue crosslinking. Keratin sits at Grade D on 2 studies and 120 participants, and it has the most specific supporting trial of the three: a clinical trial of a solubilised keratin preparation measured hair and nail parameters directly [2]. Related work has examined whether nutraceutical formulations can induce keratin expression in human follicular cells and in a randomized clinical setting [3], with one such formulation assessed against hair growth and tropism in a randomized trial [4].
Three methodological problems recur across this category. Combination products cannot attribute an effect to any single component. Nail and hair endpoints are slow, since a fingernail replaces itself in roughly six months and a toenail takes twelve to eighteen, so trials shorter than that measure a partial plate. And self-rated appearance, the most common outcome measure, is the one most sensitive to expectation.
One practical caution is analytical rather than dermatologic: high-dose biotin interferes with immunoassays that use biotin-streptavidin binding, which can distort thyroid and cardiac troponin results. Hair loss, nail dystrophy or skin change that develops rapidly warrants investigation of an underlying cause, since thyroid disease, iron deficiency and autoimmune conditions all present this way.
